As we continue to work towards having an efficient process with visitation at Bethany Village we will be launching eConfirm visitation and screening software. Effective May 17th, all visitors to the will no longer be required to call and schedule your visit. When you arrive at the Linden House reception area for your visit, you will continue to sanitize, mask, and temperature check. Then rather than signing in on paper, you will complete a screening and attestation on a computer monitor in the lobby. The first time you screen you will be required to enter contact information. For any following visits, you will only be required to complete the symptom screening and attestation. If you wish to enter this information prior to your first visit, we will provide you that information in the coming days via email and with an additional update on this webpage.

With the start of using eConfirm, we will be consolidating entrances. Also effective May 17th: when you are visiting Crescent Crossing, Pavilion, or Courtyard Park (Blossom and Cherry) and you have completed the attestation/screening in Linden House, you will then park in front of Crescent Crossing and enter through those doors and proceed to the location where your loved one resides. We wish to reiterate that visitors to Courtyard Park and Pavilion will no longer enter directly to those units from the parking lots in the back of the building. Crescent Crossing visitors will no longer be escorted through the Linden House to Crescent Crossing effective May 17th.

Announcements

On Tuesday April 27th, The Centers for Disease Control (CDC) updated their guidance to healthcare settings in relation to mask wearing and distancing for residents and staff who are vaccinated. As we have communicated with previous changes in guidance, states must wait for their state health department to also revise their guidance before any protocols or policies can be changed. With the information from CDC, Bethany Village would be able to allow vaccinated residents to eat in a communal area with other vaccinated residents without distancing or use of face masks. Vaccinated residents would also be able to attend activities with other vaccinated residents, forgoing distancing and masks. We are very encouraged that we will be able to allow these social exchanges to the residents again without the barriers of distancing and masks once the Ohio Department of Health revising their orders.  However, for your loved one to be able to participate, they must be vaccinated. Bethany Village is offering administration of the Moderna vaccine in partnership with Diamond Pharmacy, who contracts with Bethany Village on all medications in the licensed area as well as annual influenza vaccines.  The next clinic is schedule for May 20th and will be offered every 4 weeks thereafter to provide first and second doses to new staff and residents electing to be vaccinated. Please see links below for the Moderna “Vaccine information sheet” (VIS) as well as the consent forms. If you wish for your loved one to receive the vaccine, please contact the unit manager in the area they reside. If you wish to have your loved one vaccinated in a community clinic, please also communicate the arrangements to the nursing unit, so we can be sure our records reflect the resident’s vaccination status correctly.

As we are notified of revised orders from the Ohio Department of Health (ODH) which will allow us to change our current policies and procedures for dining and activities we will update the website with that information.

The current round of testing at Bethany Village has returned with all negatives. Indoor visiting will resume on March 29. Please continue to check this page for the latest announcements regarding visits. Before visiting, please call our message line, 937-436-7749 to hear the most up-to-date information.

Bethany Village received new orders from the Ohio Department of Health (ODH) which allow for visits with your loved ones in our facility. While we are excited to be once again have visitors on campus, there are protocols we must follow. At the moment, Bethany Village is unable to allow visits in the nursing home due to positive Covid cases identified in the last week. After a negative round of testing, we will be able to begin visits. Our assisted living units, Pavilion, and Crescent Crossing, are currently accepting visiting appointments.

We will provide current status of visits for all units, by contacting the message line at 937-436-7749. We recommend calling this number prior to coming to Bethany Village to ensure we can accommodate a visit on that particular day.

Here are some things you need to know about visits:

  • Recommended visiting hours are from 9 a.m.-3 p.m. everyday.
  • To schedule your visit, please contact the nursing unit 24 hours in advance of your visit.
  • We recommend visits scheduled for two hours unless otherwise indicated during scheduling.
  • Visits will be limited to two visitors at a time.
  • Recommended age of visitors is 12 years and older.
  • Pets are permitted, but must up to date on vaccinations, leashed, and attended to at all times.
  • All visitors will enter the Linden House reception area to be screened. All visitors must wear a facility-provided surgical mask at all times, and escorted to the resident’s room or outdoor visiting area by a staff member.
  • Visitors must complete hand hygiene before and after the visit and during visit as needed.
  • We encourage contact-free visits; however, if the resident you are visiting has been fully vaccinated and chooses to have close contact with you, that is permitted.
  • If you are currently receiving virtual visits and will not be able to participate in the new indoor visit offering, please contact the unit and notify them you wish to continue the virtual visits. Please be aware that with the implementation of indoor visiting, availability may change.

Visitation:

In order to follow both state and federal guidelines for general indoor visitation, a facility cannot have any new COVID cases in the past 14 days. For Bethany Village that includes our rehabilitation unit where many patients come directly from our hospital partners and may not have had the opportunity to be vaccinated as our long-term care residents have. Please keep in mind, Bethany Village is one of the largest care facilities in the Dayton area. We have more than double the number of residents, as well as many more healthcare workers and other staff than most other area nursing homes. As a facility our positivity rate is less than our county, but even one case in 14 days prevents us from starting indoor visits.

We wish to share that we will not be able to allow indoor visitation in our facility due to certain federal and state rules. Visitation is permitted at nursing homes in Ohio if the facilities meet the federal Centers for Medicare and Medicaid Services (CMS) criteria to allow visits:

  • No new onset of COVID-19 cases in the last 14 days
  • The facility is not currently conducting outbreak testing
  • CMS reports the COVID-19 county positivity rate at less than 10%

Compassionate care visits are available upon approval for certain resident conditions and situations. Our nursing unit teams screen residents for providing these special visits, but they may also be requested by residents and their families and friends. They may be approved to allow family members and others to provide comfort, support, and assistance to a resident whose wellbeing is suffering or at risk. Reasons for allowing compassionate care visits include:

  • Recent loss of a loved one
  • Transition in the area of care/location of the facility
  • Weight loss
  • New onset of symptoms such as pain and anxiety

If you have questions about compassionate care visits availability to your loved one, please contact the nursing unit team.

The website will be updated within 24 hours with any additional new cases and the cumulative review will be provided each Thursday by 5 p.m.

March 18, 2021 – We are awaiting the new Ohio orders to follow the CMS guidance changes last week. As we await the Ohio orders, we continue to provide compassionate care visits. If you are interested in compassionate care visits, please contact the nursing unit manager or social worker to discuss availability.

As we enthusiastically wait for the Ohio orders, we want to take the time to remind all future visitors of a few things related to visiting:

  • Our facility will still be required to schedule visitors. As each resident may have many that want to visit, we want to encourage all visiting parties to communicate when you wish to visit, so that there are not conflicting visit requests. Communicating this amongst all parties first, will make scheduling the visits with the facility much smoother.
  • Please keep in mind that our staff will be providing care, as per normal routine. So during your visit, your loved one may be scheduled for therapy, medication administration, or other care needs. Please be respectful when these interruptions occur during your visit.

The CDC announced the people who have been vaccinated do not need to wear masks in certain situations- however this does not apply to healthcare settings. So when visiting with your loved who has been vaccinated, you still must wear a mask, even if you have been vaccinated as well.

Since our last update on March 9th, we have had no additional nursing home staff cases and no additional assisted living staff cases. There are no additional nursing home resident cases and no additional assisted living resident cases.

Hello Families, Friends and Loved ones of the Bethany Village residents,

We recognize there are many “terms” used to refer to different ways to visit in our facilities. We wanted to share some of the definitions and expectations of each type of visit we are permitted under the direction of the Ohio Department of Health (ODH). Yesterday, March 10th, Centers for Medicare and Medicaid (CMS) updated their direction considering the vaccination status of residents. We will now be advocating for this at a state level with ODH so that we can implement reduced measures that CMS communicated via their revised memo yesterday. This includes reduced needs for social distancing during visits. As soon as we have updates in the orders from the ODH we will share via a website posting.  At this time with the current ODH orders visit standards are listed below.

Visit Expectations

Compassionate Care Visits- available currently in Assisted living and Nursing Home

  • Reviewed in advance to ensure resident meets criteria to receive.
  • Scheduled in advance and cannot be routine or regular.
  • Masks are to be worn properly (covering you mouth and nose) throughout the entire visit.

  • Social distancing (6 feet) must be maintained at all times.
  • Please refrain from hugging, hand holding or other behavior where social distancing cannot be maintained.
  • Eating and drinking by visitors during a visit is not permitted.
  • Visitors will be escorted to and from the resident’s room.  At the conclusion of your visit remain in the resident’s room and turn on the resident’s call light.  This will alert a staff member that your visit has concluded and the staff member will escort you from the resident’s room.  Do not leave the resident’s room prior to the staff member’s arrival.

Indoor Visits- currently available in assisted living

  • Scheduled in advance.
  • Limited to a maximum of two visitors per visit and limited to thirty minutes.

  • Please refrain from hugging, hand holding or other behavior where social distancing cannot be maintained.
  • Eating and drinking during the visit is not permitted.
  • Visitors will be escorted to and from the visiting area.  At the conclusion of your visit you will be escorted from the visiting area.  Do not leave the visiting area prior to the escort’s arrival.

Outdoor Visits- Currently available in Assisted living

  • Scheduled in advance.
  • Limited to a maximum of two visitors per visit.
  • Masks are to be worn properly (covering you mouth and nose) throughout the entire visit.
  • Masks are to be worn by both the resident and the visitor(s).
  • Social distancing (6 feet) must be maintained at all times.
  • Visitors must be 12 years of age or older.
  • Please refrain from hugging, hand holding or other behavior where social distancing cannot be maintained.
  • Eating and drinking during the visit is not permitted.

Virtual visits and window visits are also available to residents of both assisted living and nursing home. These types of visits do not require any type of screening but in some cases where staff support is needed do need to be scheduled in advance. Please contact the nursing unit your loved one resides for more information.

For all types of visits, if, at any time, the above expectations are not adhered to the visit for the day will be ended.  Non-adherence to the above may impact the future visits.

Thank you for your cooperation in keeping our residents and staff safe.

**Please note the following communication was sent out via email on 2/1/21 to nursing home contacts:

Dear Bethany Community Member,

As many of you are aware, residents in our nursing home began receiving the COVID-19 vaccine in December of last year.  Our partner pharmacy for the vaccine, CVS, is preparing to complete their third, and final, vaccination clinic here at Bethany Village.  The last vaccination clinic is being held this week.

It has been the hope of many of you, as well as our team members here, that vaccination of our residents would equate to indoor visitation.  At this point in time, the Centers for Medicare and Medicaid Services (CMS) continues to require that there be “no new onset of COVID-19 cases in the last 14 days and the facility is not currently conducting outbreak testing” in order to offer indoor visitation.  At this point in time the federal nursing home regulations prevent us from being able to offer indoor visitation.  We continue to offer alternate means of visitation, including regular virtual visits, the opportunity to schedule a window visit, as well as compassionate care visits.

Our advocacy organizations, such as Leading Age Ohio, as well as Bethany Village, continue to advocate for you and on your loved ones behalf.  We continue to hope and pray, along with each of you, for the day in which indoor visits are a regular part of you and your loved one’s life.

FAQ about the vaccine

What is the vaccine and how does it work?

Right now there are two approved vaccines: one from Pfizer and one from Moderna. Both require two injections 3-4 weeks apart. The Pfizer vaccine is 95% effective and Moderna is 94% effective.

The mRNA technology gives instruction to our cells to create a harmless piece that resembles the “spike protein” found on the surface of the COVID-19 virus. When our body encounters the COVID-19 virus, the antibodies recognize it and know to fight it. Neither of the vaccines contain COVID-19.

How do we know the vaccine is safe?

The first thing to note is that neither the Pfizer or Moderna vaccine contain COVID-19. You cannot get COVID-19 from being vaccinated. Secondly, the technology used to develop the vaccine has been used safely in cancer treatment for over a decade. Lastly, the vaccines were developed by following all normal protocols and with all the same oversight as any other vaccine produced for the market and approved by the FDA.

Why should I get vaccinated?

The vaccine will protect you and your loved ones. Being vaccinated protects our residents and the community at large. The benefits of the vaccine for you and the community far outweigh the short-term discomfort.

When does the vaccine start working and how long does it last?

The vaccine is only effective if you receive both dosesIt is important to receive the same vaccine for both doses. At the moment, we do not know how long the vaccine is effective. We may need to have vaccines for COVID-19 on a regular basis, like our annual flu shots. Long-term research is currently being conducted.

What are the side effects?

Immediately following the injection you can expect to have short-term discomfort: fatigue, headache, muscle aches, chills, fever, and pain at the injection site. The short-term discomfort is a good thing; it means the body is producing antibodies.

Will Bethany Village staff be receiving the vaccine?

Yes. While we are not requiring staff to receive the vaccination, we are encouraging them to get it.

Do I still have to wear a mask?

Yes. Until a very large number of people receive both doses of the vaccine, it will be important for everyone to continue to care for each other by wearing a mask.

When will I be able to visit my loved one in the nursing home again?

We do not know yet. In order for the vaccine to stop the spread of COVID-19 and achieve herd immunity, lots of people will need to be vaccinated. The sooner people get vaccinated, the sooner we can allow some form of visiting.

What is the best source for more information?

It is important to gather information from experts. Social media has lots of opinionated-yet-uninformed commentators. A single meme or graph cannot capture the complexities of scientific research and the process for creating and distributing a vaccine for millions of people. Here are some links to well-researched, fact-based information from experts in the field:

We want to make sure everyone understands the updates we post. Below is a brief Q&A regarding the numbers we are posting in compliance with state guidelines.

The website gives cumulative cases since March. How many cases do you have right now?

You’re right—the numbers on our website are cumulative since March. We do not have 80 active cases right now. Active cases could be figured by looking at the last two weeks of updates. Over the past two weeks we have 20 new cases—four residents and sixteen staff.

Testing is ongoing and individuals’ medical information is protected by HIPPA. However, we can make some educated guesses by looking at the number of cases from November 12 until now. Since November 12, we have had four residents test positive and sixteen staff test positive.

That seems like a number I should worry about.

To be clear, we worry about any number greater than zero. But, it is also important to put this in perspective—Bethany Village has 720 residents and 630 employees. If we do the math, we see the current positivity rate for Bethany Village residents is 0.5%. The positivity rate for staff is 2.5%. As of November 25, Ohio has a positivity rate of 13.5%, while Montgomery County sits just over 17%. Bethany Village as a whole (both staff and residents) has a current estimated rate of 1.5%. The number is low because we have taken action to keep everyone—staff and residents—as safe as possible.

Is the situation getting worse?

From the beginning, we have taken the threat of COVID very seriously. We have been able to consistently trend much lower than the state and surrounding areas. The number of cases at Bethany Village over the past few weeks is a small increase compared to the spike seen in Ohio. We’ve been able to trend lower because we have been diligent in our efforts to keep everyone safe. So, yes, the pandemic is getting worse everywhere, but the effect at Bethany Village is far less than any of the surrounding areas.

October 29, 2020 – The Ohio Director of Health mandated biweekly COVID-19 testing for all staff working in Assisted Living programs. In order to stay in compliance, Bethany Village has been testing 90-100 staff in each round of testing. So far we have conducted three rounds of testing. In our most recent round of testing, one Assisted Living staff member tested positive. The staff member did not display any physical symptoms and has since been quarantining.

As we move forward, we will report our COVID-19 cases in four categories: Assisted Living Resident, Assisted Living Staff, Nursing Home Resident, and Nursing Home Staff. The updated numbers are listed below.

Updates to the Public Health Advisory System are made on Thursdays. To coincide with the information provided by the Governor and the Ohio Department of Health, we have decided to post our weekly updates on Thursdays by 5 p.m. going forward. July 8, 2020 – As of today, July 8, 2020, we have no positive COVID-19 resident cases and one new positive COVID-19 staff case.

Yesterday Governor DeWine announced a mandatory mask policy for Ohio counties classified as Level 3 under the state’s Public Health Advisory Alert System. The policy goes into effect today, July 8 at 6 p.m. Montgomery, Hamilton and Butler counties are amongst the counties classified as Level 3.

We also want to share a chart developed by the Texas Medical Association showing the levels of risk associated with various social activities. (download pdf of chart).

We understand that families are conflicted by the current situation—they want to visit their loved ones in-person, yet are concerned about the spread of COVID. Due to the current Level 3 Alert in Montgomery County, we will not start visitations on July 21. We will continue to monitor the situation in our area and keep families informed as developments occur.

July 1, 2020 – As of today, July 1, 2020 we have no positive COVID-19 resident cases and no new positive COVID-19 staff cases.

June 26, 2020 – As of today, Friday, June 26, we have no residents testing positive for COVID-19.

This week we received the results of the nursing home staff testing for COVID-19 conducted by the Ohio National Guard.  More than 400 nursing home staff have completed testing this week.  Out of the 400 tests, three staff tested positive yet exhibited no symptoms. These three staff have been removed from the schedule and will not be returning to our campus until released by a physician. As we all know, even by taking every precaution known to contain this virus, it is invisible and some people do not show any symptoms.

We want to thank the Ohio National Guard and Dayton Children’s for their assistance. We also want to thank our staff, our residents, and their families for their diligence and patience during these challenging times.

June 24, 2020 – Bethany Village remains fully committed to maintaining a safe living and working environment. Below is a timeline of recent events and communications:

Based on Governor DeWine’s order in late May, and at our request to have testing completed, our nursing home staff has undergone COVID-19 testing. We have been working with the Ohio National Guard for the implementation of staff testing and Dayton Children’s Hospital is processing them. As of today, more than 400 staff have been tested.

Testing is mandatory. Staff who have been on scheduled family leave or vacation are required to have testing completed at an area testing site before being scheduled to work. Printed documentation of the test results are required.

We anticipate being notified of only positive test results and so far we have not received any calls. While all of our staff have remained vigilant since late January, we know that despite all of our efforts, this invisible virus may have infected staff who do not exhibit any signs of illness (asymptomatic).

We appreciate the due diligence our staff has taken to stay healthy and to care for our residents.

June 19, 2020 – Based on an order from Governor DeWine in late May, we have been informed that members of the Ohio National Guard will be on Bethany’s campus Monday, June 22, to provide COVID-19 testing of staff.  This is a routine assessment and includes the staff in the nursing home and assisted living areas of our campus.

We welcome the State of Ohio’s assistance in fighting this invisible health crisis. This does not change the diligence our healthcare teams have demonstrated since late January and will continue to remain vigilant as we move forward together.

June 17, 2020 – As of today, June 17, 2020 we have no positive COVID-19 resident cases and no positive COVID-19 staff cases.

June 10, 2020  – As of today, June 10, 2020 we have no residents testing positive for COVID-19. Also, one staff, who has been off campus since May 23, 2020 did test positive while out. They will not be returning to campus until cleared by a physician.

June 3, 2020 – As of June 3, 2020 we have no active COVID-19 cases among residents or staff at Bethany Village.

On May 30, 2020 we shared that a resident had tested positive for COVID-19. We consulted with public health and were advised as a proactive precaution to test residents and staff in one area of the nursing home. Testing occurred on Monday and Tuesday of this week. All resident COVID-19 test results were negative. Staff tested thus far in that area of the nursing home have also tested negative.

We will be posting all future updates to this website only. Please check back to this page for the latest information and to see all updates. We will post weekly, on Wednesday’s, by 5:00 pm.

May 30, 2020 – Today, we received confirmation that Bethany Village has our first nursing home resident testing positive for COVID-19. The resident did not display any symptoms of COVID-19 but was required by a local healthcare provider to be tested as a precautionary measure to receive a procedure in the community. The resident continues to be symptom free.  We responded immediately by placing the resident in isolation.  Following our normal procedure, the resident has worn a face mask when out of their room. At this time no other resident has demonstrated any symptoms. All staff wear masks at all times when in any resident care area. The public health department has been notified and we are performing contact tracing to identify any other people with whom this resident may have had direct contact. The staff has exceeded the standard symptom monitoring with resident symptom checks which includes temperature and oxygen level checks.

We thank all our staff for their continued diligence in maintaining the health, wellness and safety of our residents as well as each other by continuing to follow CDC and health department guidelines. We recognize that this is a highly contagious virus that could impact us despite our best efforts. We remain vigilant. Just a reminder we will continue to be transparent and report any positive Covid-19 cases but due to the privacy of our residents and staff we will not release names of residents or staff nor where they live or work.

May 26, 2020 – As of May 26, 2020, no Bethany Village resident has tested positive for COVID-19. Other than the one staff reported last week, no additional staff have tested positive for COVID-19. We thank all our staff for their continued diligence in maintaining the health, wellness and safety of our residents as well as each other by continuing to follow CDC and health department guidelines. We recognize that this is a highly contagious virus that could impact us despite our best efforts. We will remain vigilant and continue to update you on key developments. Just a reminder we will continue to be transparent and report any positive Covid-19 cases but due to the privacy of our residents and staff we will not release names of residents or staff nor where they live or work.

As more and more businesses and offices begin to re-open, we want to remind everyone of  the importance of following CDC safety tips. CDC strongly recommends everyday preventative steps, including wearing cloth face masks in public spaces, washing your hands frequently, and practicing physical distancing by keeping six feet of space between yourself and other people outside your household. We appreciate your ongoing efforts to stay healthy and help to slow the spread of COVID-19.
